Liberia: EPA Fines Sethi Ferro Fabrik $5,000, Shuts Down Oxygen Plant After Fatal Explosion

Monrovia –The Environmental Protection Agency (EPA) has fined Sethi Ferro Fabrik Steel Factory $5,000 following a fatal explosion at its oxygen plant in Gardnersville on February 17, 2025.

The blast, which was caused by an oxygen cylinder explosion, resulted in the death of Saah J. Samukai, an employee at the facility.

An investigation by the EPA revealed significant safety violations at the plant, including inadequate fire safety measures, improper handling of gas tanks, missing emergency exits, and expired environmental permits. 

As a result, the EPA ordered the immediate shutdown of the facility on February 19, 2025.

While the EPA recognized the plant’s contribution to supplying free medical oxygen to hospitals, it emphasized the importance of safety compliance. 

The agency has stated that a temporary 90-day operating permit will be issued once the fine is paid and safety improvements are made. However, a full environmental audit must be conducted before the plant can resume normal operations.
